Many adults considering Invisalign have already had dental work done. Perhaps you received a crown after a root canal, replaced a missing tooth with an implant, or have several restorations that have been in place for years. Now you would like to straighten your teeth, but you are not sure whether those restorations make orthodontic treatment more complicated.
The good news is that having crowns or dental implants does not automatically prevent you from getting Invisalign. However, these restorations behave differently during orthodontic treatment, and understanding those differences is essential when we plan your care. What matters most is the condition of your teeth, the type of dental work you have, and the changes you want to achieve.
A dental crown covers a natural tooth, but the root remains underneath. Because the tooth is still supported by a periodontal ligament, it can often respond to orthodontic forces much like a tooth without a crown. This means Invisalign may be able to move a crowned tooth, provided its root and supporting tissues are healthy enough for the planned movement.
Consider an adult who received a crown on an upper front tooth after an accident several years ago. At the time, the restoration matched the neighbouring teeth, but gradual changes in alignment eventually made it appear slightly crooked. The patient might assume the crown prevents orthodontic correction, when the more important consideration is the condition of the natural tooth underneath. We would examine the root, surrounding bone, and restoration before deciding whether movement is appropriate.
One practical difference involves Invisalign attachments. These small tooth coloured shapes are sometimes bonded to teeth to help aligners produce specific movements. Bonding them to porcelain or other restorative materials can be more challenging than bonding to natural enamel, so we may need to adjust how attachments are used. We also consider whether the crown’s existing shape will still look appropriate after the tooth changes position, since Invisalign cannot alter the restoration itself.
Why Dental Implants Require A Different Approach
Dental implants are different because they cannot be moved with Invisalign. Unlike natural teeth, implants are integrated directly into the jawbone and do not have the periodontal ligament that allows orthodontic movement. Once an implant has integrated, its position is essentially fixed.
This does not mean someone with an implant cannot straighten their natural teeth. Instead, we need to plan around the implant’s existing position. For example, an adult with an implant replacing a lower molar may still be a good candidate for Invisalign to correct crowded front teeth. However, if the implant occupies space needed for a planned movement, it can limit what we can accomplish.
Front tooth implants can be particularly interesting because natural teeth may continue to shift over the years while the implant stays in place. A patient may notice that an implant supported crown no longer lines up perfectly with the neighbouring teeth or that the gum levels look uneven. Moving the natural teeth may improve some aspects of the smile, but it cannot reposition the implant itself. We need to establish which changes are achievable before recommending treatment.
What Else Can Affect Your Invisalign Treatment?
Crowns and implants are only part of the dental history we consider. Adults may also have bridges, previous periodontal treatment, or teeth that experienced trauma years ago. These factors can influence the safety and predictability of orthodontic movement.
During an assessment, we pay particular attention to:
- Gum health and bone support. Teeth need adequate supporting tissues for orthodontic movement. If someone has active periodontal disease, that condition generally needs to be treated and controlled before Invisalign begins.
- Fixed dental bridges. Bridges connect dental units, which can restrict independent tooth movement. We examine how the bridge is supported and whether its design affects the proposed correction.
- Previous dental trauma. A tooth injured many years ago may have changes involving the root or supporting tissues that are not obvious from its appearance. We consider that history when evaluating orthodontic suitability.
- Existing dental problems. Decay around a crown, a damaged restoration, or another unresolved dental issue may need treatment before aligners are introduced.
- The movements required. Some corrections involve straightforward repositioning, while others require more demanding rotations or changes in angulation. We assess whether the intended movements are suitable for clear aligners.
These findings help us determine whether Invisalign can begin with your current dental work or whether another dental concern should be addressed first. If you have records from previous implant placement or major dental procedures, bringing them to your consultation can be helpful, although they are not necessary to schedule an initial assessment.
When Invisalign Should Come Before A New Crown Or Implant
Sometimes the most important question is not whether Invisalign can work around an existing restoration, but whether orthodontic treatment should happen before new dental work is completed. This is especially relevant when teeth have shifted around a missing tooth or when several visible crowns are being replaced.
Imagine a patient who lost a lower molar years ago and is now considering an implant. The neighbouring teeth may have tilted toward the empty space, leaving less room for the replacement. In an appropriate case, Invisalign may help reposition those natural teeth before the implant is placed, allowing the restorative dentist to work with a more suitable arrangement.
Something similar can happen when older front crowns were designed to disguise crooked teeth. Replacing those crowns immediately may mean creating new restorations around the same alignment problem. If orthodontic treatment is appropriate, moving the natural teeth first may allow the final crowns to be designed with better proportions and positioning.
This is why we encourage adults to mention any future dental work during their orthodontic consultation. When necessary, we can coordinate with the restorative dentist to establish the appropriate treatment sequence. Planning both procedures together can make a meaningful difference to the final result.
